First its important to now that Jerry's "tortures" are self imposed as he himself has chosen to do this. Most noticeably physical discomfort is that his nose is bleeding profusely and is constantly in pain. Mentally he is in a state of discomfort, confusion and uncertainty. He also feels lonely and alienated from others.  
Jerry's mental torture begins as he prepares himself, imaging what he needs to do to succeed. The fear of not having enough breath to survive the swim through the tunnel was to be the most supreme torture, so Jerry prepares himself physically by holding his breath for long periods of time.
