Carol Okasinski, 56 yr old is a new patient. She was a patient of another physician in the community, however his receptionist was often rude to her, and she decided no to go there anymore. Mrs. Okasinski is obese and has hypertension, type 2 diabetes, osteoarthritis in her hands and knees, and problems with depression. While filling in the health history form, she says she cannot fill in the names of the medications she is taking. She is on a lot of medications prescribed by her previous physician. She says she could not get the chilproof pill containers open because of the arthritis in her hands, so she had her husband throw away the childproof containers and transfer each medication into an easy-to -open plastic container. She knows when to take her medications, but she does not know the names of them or why she is taking them. She has brought in a bag all her medication in their plastic containers.
