(C/O) complaint of

CXR Chest X Ray to R/O rule out

Patient “discontinued” on “over the counter” meds “no known drug allergies”

Vital signs QID “4 times a day” notify MD “medical doctor” immediately for vomit or heartburn

HX “history” disease of the heart muscle, complaint of I’m assuming hyper gastric is the same as epigastric which would be in upper middle quadrant of the abdomen, vomiting, high heart rate, endoscopy is a minimally invasive long thin tube that is inserted either through the mouth or anus to look at the internal organs to rule out a gastric ulcer
