I think the answer is C. The first NA did not properly turn off the water, and the second NA did not properly use paper towels.  

When you are to close off the sink you use the paper towel used to dry your hands. When the first NA opened the sink the hands where dirty, the NA then proceeded to close the sink with the washed hands.... now the hands are contaminated yet again.

The second NA used the paper towel to wash AND dry the hands. You are supposed to use soap for 30 seconds and then rinse for 30 seconds. The towel is to be used only when drying and not washing.
