The licensee shall notify the Department, in writing, within ____ days of a change of administrator, which includes information regarding: the name, and residence and mailing addresses of the new administrator; the date he/she assumed his/her position; and a description of his/her background and qualifications, including documentation of the required education and administrator certification.
