Pt with 2 hrs of intense, constant chest and neck pain radiating to the back. On exam there is an early decrescendo diastolic murmur heard at the R sternal border of the 4th intercostal space. EKG shows LVH, and T wave inversion in leads V5 and V6. Dx? What is the murmur? How is this diagnosed? Tx?
