Harassment is an unfortunate aspect of life in today’s workplace, but when it occurs it cannot be tolerated. The negative impact it has on employees and company culture makes it imperative that companies don’t turn a blind eye to it.

Your boss comes to you after attending a conference on dealing with harassment in the workplace and tells you to prepare a form that employees can fill out when they want to file a complaint about inappropriate behavior in the workplace that they have either personally experienced or witnessed. Since your boss attended the conference and you did not, you will need to do a little research about what to include in the form.

For this activity, you will anticipate what information will be needed from victims of, and witnesses to, harassment or discrimination in the workplace in order for the company to respond with the appropriate disciplinary or legal action.

Your completed form should include the following information:

Research sample complaint forms that human resources departments use for their companies.
Draft a form that employees can use to report behavior they believe constitutes harassment or discrimination. The person filling out the form will be either the victim of the harassment or discrimination or a co-worker who has witnessed the event, so the form should request information about how the person filling it out is involved or how they know about the misconduct.
Be sure to include a place on the form where the person submitting the complaint verifies that the information they have provided is true and accurate.
